Months after the pandemic, Andrea's ward is reopened but someone is missing: Lorenzo is no longer there since he died in March 2020 due to COVID-19. Andrea left the ward to visit elderly people outside the hospital, occasionally helped by Riccardo. The hospitalization of Giacomo, a health worker, after what appears to be a hysterical attack is the first case under the direction of the new Chief. Lorenzo's death causes create concern, and an investigation threatens to reveal that he was patient zero. Meanwhile, Gabriel still intends to leave Italy. After losing her child, Giulia is increasingly determined to leave for Genoa once the investigation is over. Damiano Cesconi arrives in the department to replace Lorenzo. Doc decides he wants be the Chief once again and dreams of suffocating Lorenzo to the point of killing him, although it is not clear if it is only a dream or there is something true...
